Neverending OAB issues, new users never appear unless i delete .oab files in EVERY users outlook porfile. Sync errors, HELP

We have had EXchange 2010 stock clean install out of the box since January, hoping the default settings would finally bring us a nice healthy exchange after recent mgration from 2007. NOT HAPPY :-(

My biggest headache is the OAB. I have run through all the guides on how to recreate, regenerate, redistribute the OAB, and before I go tinkering with IIS settings, I need some qualified guidance. It was working about a month ago after the same issue appeared and doing the above we finally started seeing new users in OAB/cached mode.

Then I started seeing the dreaded "Microsoft Exchange offline address book,   0X80190193" in my Outlook sync log, and I have tried everything and the only fix/workaround is to manually delete users .oab files on their pc. This is not a solution, but confuses me more. Because obviously users can connect to OAB and download fresh copy if i delete their .oab files, but after the first time those .oab files never update again. Manual Address book downloads NEVER work.

Before i go fiddling and changing intricate permissions/auth settings in IIS and possibly mess up more stuff and start makinga  mess of an otherwise clean install, I need to get to the bottom of OAB issue. Its such a fickle bastrd ye 'ol OAB, why oh why?!?!?!?

My last/safe/clean effort was completely removing the OAB from EMC, the also removing/deleting the virtual directory. Then recreating both cleanly using the shell's default setting. Nope. Exact same problem.
