Meeting being randomly forwarded

I am having some weird and inconsistent issues with meeting requests that have mostly been occurring in the IT department.  We are running Exchange 2010 in a DAG config with the MBX, HT and CAS roles all on the same server.  We are also running Outlook 2010 as our mail client.  We have 3 directors, the CIO and a woman that acts as an admin for all of them.  The admin is a delegate for all of the directors and the CIO.  

The latest issue is this.  One of the directors (Sarah), sent a meeting request to Kelvin.  The meeting request was sent and accepted from laptop PCs running Outlook 2010.  In IT, our users are pretty good with not doing anything relating tot he meetings from their mobile devices (iphone, BB and iPads mostly).  All was good up until this point.  Several hours later, one of the other directors and the admin received an invitation that says that it was forwarded from Kelvin for them to attend the meeting.

Problem is, Kelvin says that he didn't open or alter the meeting in any way since accepting it and did nothing to invite these other participants.  Same story from Sarah and their admin.  I'm looking for a methodical way to troubleshoot these types of meeting issues to try to instruct users on the best way to create and deal with meetings to avoid problems.  As I said, right now, all of our users have been instructed to do all meeting realted activity from their PC on the Outlook 2010 client.  

As background, Kelvin also has a MAC with Outlook 2011 installed, an iPad and iPhone, and Sarah has a BB and iPad.  Thanks for any help you can provide.
